Job Description

The Support Staff provides comprehensive case management services to individuals receiving residential and outpatient mental health and substance use treatment. This role supports clients in accessing community resources, coordinating care, developing individualized service plans, and removing barriers to recovery. Will work collaboratively with clinical staff, community partners, and support systems to promote client stability, wellness, and long-term recovery. This position requires employees to be at least 18 years old. This role may require working nights and weekends shifts required staff to be on site overnight.
Duties may include:
Develop, implement, and monitor individualized service plans in collaboration with clients and treatment teams. Coordinate care with therapists, medical providers, probation/parole officers, family members, and community agencies as appropriate. Provide referrals and linkage to housing, employment, medical care, transportation, insurance, and other supportive services. Assist clients with benefits applications (Medicaid, SSI/SSDI, SNAP, etc.). Monitor client progress toward treatment goals and adjust service plans as needed. Provide ongoing encouragement and motivational support to clients in early recovery. Coordinate with clinical staff, case managers, and other service providers as needed Maintain accurate, timely, and compliant documentation in electronic health records. Ensure services are delivered in accordance with state, federal, and agency regulations. Transport clients in a company vehicle Meal prep with clients
Qualifications Background Check:
Required. Candidates must consent to a criminal background check to be considered.
Driver's License:
Clean and valid driver's license required. CPR and First Aid Certification, Narcan Administration Certification Food Handler's Permit Utah Case Management Certification (willing to obtain)
Experience:
Prior experience in residential support, recovery services, or behavioral health settings preferred but not required.
Communication Skills:
Strong interpersonal and verbal communication skills required.
Reliability:
Must be dependable, punctual, and able to work flexible hours, including nights and weekends.
