- Temporary, Full-time, Contract, Internship New Growth Behavioral Health provides high-quality behavioral health services in the natural community setting to individuals in need of essential quality care in order to stabilize their mental health and deal with behaviors driven by their underlying conditions.
Duties and Responsibilities include:
- Provide mental health treatment to individuals receiving services (services may include Intensive In-Home, Mental Health Skill-Building or Crisis Stabilization discussed during the interview) via behavior management, modeling, care coordination, crisis intervention, psychoeducation, and skills training.
- Manage a caseload of clients.
- Provide services in community locations where the individual lives, works, participates in services, or socializes. Locations include schools, homes, places of employment or education, or community settings.
- Work with clients to foster the development of new improved coping, problem solving, communication skills, case management, etc. and any other goals as directly related to the client's individualized service plan.
- Establish a good working relationship with other public and private agencies/services and interface collaboratively with them (i.e., Social Services, CSBs, hospitals, etc.)
- Communicate and coordinate effectively with other members in clients' care team, including case managers, psychiatrists, medical providers, family members, and other service providers at least once per quarter to ensure coordination of care.
- May provide supportive assistance in the areas of activities of daily living, money management and other basic physical needs as it relates to their independent living skills.
- Linkage and referral to ongoing services, supports and resources(such as health services, recreational activities, housing, financial resources, food pantry, government entitlements, fuel assistance, SNAP benefits, etc.) as appropriate and least restrictive level of care.
- Participates in the development and implementation of service plans, objectives and methods necessary for providing effective services to individual clients and families.
- Attend weekly staff meetings, clinical supervision, and trainings.
- Complete required clinical documentation and maintain client records by documenting clients' progress (i.e., progress notes, quarterly progress reports, and updated treatment plans)
- Evaluate clients' participation and progress on goals and update Individual Service Plan at least once each quarter to reflect progress or address barriers to progress, complete quarterly reports, progress notes, and other necessary documentation.
- Ensure compliance with regulations set by governing bodies (Medicaid, CARF, DBHDS) and maintain standards of care according to NGBH policies and procedures.
- Any other job-related tasks as assigned by the supervisor.
