Step Above Counseling Services is seeking a compassionate, organized, and dependable Case Manager to join our team. The Case Manager will support clients by coordinating services, connecting them with community resources, monitoring progress, and collaborating with members of the client's care team. This position is ideal for someone who enjoys helping people navigate systems, solving problems, building supportive relationships, and making a meaningful difference in the lives of others. What You'll Do Review and audit documentation daily for accuracy, completeness, and compliance. Work directly with clients to identify needs, goals, and barriers to services. Help clients access behavioral health, healthcare, housing, employment, transportation, benefits, and other community resources. Coordinate referrals and follow up with providers and community agencies. Maintain regular communication with clients and members of their care team. Participate in treatment team meetings and collaborate with counselors, therapists, care coordinators, and other providers. Monitor client progress and help ensure services remain aligned with individual goals. Advocate for clients and assist them in navigating healthcare and community-based systems. Identify concerns or changes in client needs and communicate them to the appropriate clinical or supervisory staff. Complete accurate and timely case notes, service documentation, and other required records. Maintain client confidentiality and follow HIPAA and organizational requirements. Attend staff meetings, supervision, training, and professional development activities. Perform other duties as assigned. What We're Looking For Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field required. Previous experience in case management, behavioral health, social services, care coordination, or a related field preferred. Strong communication and interpersonal skills. Excellent organization and follow-through. Ability to manage multiple clients and priorities. Comfortable working independently and as part of a team.
