Working alongside an interdisciplinary team, you will provide spiritual support that honors each individual's beliefs, values, and traditions while helping patients and loved ones find peace, connection, and dignity throughout their hospice journey. Your Impact Conduct spiritual assessments and ensure appropriate spiritual care services are provided in a timely manner. Participate in comprehensive patient assessments and individualized care planning to address identified spiritual needs. Provide direct spiritual support, counseling, guidance, and comfort to patients and families/caregivers. Collaborate with and serve as a liaison to community clergy, chaplains, and spiritual counselors. Attend interdisciplinary team meetings and patient care conferences as an active member of the care team. Provide education, consultation, and support to team members regarding spiritual care needs and concerns. Maintain accurate and timely documentation of spiritual care services and related activities. Recruit, develop, and maintain relationships with community spiritual resources to support patient and family needs. Assist with the recruitment and supervision of spiritual care volunteers when assigned. Participate in quality improvement initiatives and program development activities. Provide funeral or memorial services upon request. Coordinate periodic memorial services for families, staff, volunteers, and community clergy. Perform other related duties as assigned in support of quality hospice care. Requirements Graduate of an accredited seminary or school of theology with a Bachelor of Divinity (BD), Master of Divinity (MDiv), or equivalent theological degree. Ordained, commissioned, endorsed, or credentialed according to the practices of a recognized faith tradition, or possess equivalent theological training. Comfortable providing spiritual care in an interfaith environment and able to respectfully support individuals from diverse backgrounds, beliefs, cultures, and traditions. Knowledge of hospice philosophy, end-of-life care, grief support, and bereavement services. Hospice, healthcare chaplaincy, pastoral care, or related experience preferred. Clinical Pastoral Education (CPE) training preferred. Valid driver's license, reliable transportation, and proof of automobile insurance. Ability to maintain accurate documentation and comply with applicable hospice regulations and standards.
