As the spiritual care counselor, is to meet the patient (and their family) on their journey and help provide spiritual support.

• Formal Education in social work, seminary, theology or ordained minister is

REQUIRED.

Experience working with death and dying for individuals/family/caregiver strongly preferred. Participate on the IDT to develop and carry out spiritual care plans that respect patients' rights and preferences. Conduct regular home visits and telephone outreach to assess the emotional and spiritual needs of patients. In partnership with the branch director, provide oversight of the bereavement program to ensure bereavement and grief support meet the needs of those we serve and meet compliance requirements. Make periodic bereavement calls and share additional resources with families based on the needs Facilitate memorial services and support groups as needed. Ensure a compassionate and respectful environment for all patients and their families. Maintain accurate medical records and documentation in compliance with HIPAA regulations.
