Job Description

We Are Inspired to Serve. Join us! The Hospice Chaplain is responsible for the provision of spiritual care services to patients, families/caregivers and staff of CovenantCare Home Health and Hospice, either directly or through coordination of care with other spiritual counselors. Provides direct spiritual care to patients, families/caregivers, and staff fitting to their unique needs and in a timely manner. Where needed, lead/facilitate caregiver support groups. Assures spiritual assessment and development of individualized spiritual plan of care for patients/caregivers in the hospice or home health program. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Serves as liaison and support to community chaplains and spiritual counselors. Document direct service and ongoing communication with community chaplains and spiritual counselors. Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. Provides consultation, education, and support to the interdisciplinary group on spiritual care. Provides funeral or memorial services as requested/ needed for patients, families, and hospice team.
Experience, Credentials, and Qualifications:
Completion or commitment toward 1-2 units of Clinical Pastoral Education (CPE) or equivalent clinical training, preferred. Board certification (or progression toward) by a national professional spiritual care organization, preferred. Experience working with death and dying, individuals/family/caregiver. Comfort in an interfaith setting. Hospice experience preferred. Knowledge of and commitment to hospice philosophy of care.
Compensation Pay Range:
$80,591.00 - $113,388.50 per year Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $80,591.00 - $113,388.50 per year. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
