Essential Duties and Responsibilities:

Promotes consistent, high quality patient care by functioning within Coastal Hospice IDT model in meeting patient/family goals and maintaining effective relationships with contracted facilities Provides assessment, planning, intervention and ongoing evaluation of spiritual needs to meet assigned patient and family goals by: Attending and actively participating in scheduled meetings of the designated IDT Collaborating with members of the IDT on ways to support the patient and/or family Participating family meetings to address patient/family needs Providing spiritual support to patients/families to alleviate suffering and facilitate acceptance of the dying process Utilizing community resources to assist the patient/family to meet goals Coordinating with area clergy and religious leaders to provide spiritual care to hospice patients and families Referring patients and/or families to outside agencies, if necessary Maintains continuity and consistency of care by: Completing spiritual documentation in the patient's electronic medical record within the prescribed time frame Triaging essential information to other colleagues to communicate spiritual needs as it relates to transitions in patients care In collaboration with Bereavement Services, Spiritual Counselors will assist with providing follow-up bereavement calls, required visits and initial assessments per policy Develops and provides educational programs to Coastal Hospice staff and volunteers, local faith groups and community organization, as requested Provides spiritual support to Coastal Hospice staff, as requested and appropriate

Education and Work Experience Requirements:

Master's degree in divinity or theology or pastoral counseling from an accredited seminary or school of theology preferred Current ecclesiastical endorsement preferred Completion of a minimum of one (1) unit of Clinical Pastoral Education (CPE) within prescribed amount of time or equivalent clinical training and experience preferred Demonstrated ability in facilitating individual and group support Sensitivity to the particular needs of those in grief

Ability to safely and successfully perform the essential job functions consistent within the guidelines of ADA, FMLA, OSHA and other federal, state and local laws Must be able to lift and carry up to fifteen (15) pounds with or without the use of assistive devices Understand and comply with Coastal Hospice's mission, values, vision, policies and the Employee Handbook
