Hospice Spiritual Care Counselor Butterfly Hospice - 5.0 Encino, CA Job Details Part-time 1 day ago Qualifications Bachelor's degree Full Job Description Hospice Spiritual Counselor / Chaplain We are a new and growing hospice looking for a compassionate and dedicated Spiritual Counselor / Chaplain to join our team. We are looking for someone who is reliable, punctual, responsible, and eager to grow with us as we continue to build our hospice. The ideal candidate is caring, professional, and passionate about providing spiritual and emotional support to patients and their families during end-of-life care.
Responsibilities:
Provide spiritual and emotional support to hospice patients and their families. Complete spiritual assessments and develop individualized plans of care. Provide counseling and support based on each patient's individual beliefs, values, and preferences. Participate in interdisciplinary group (IDG) meetings. Complete visit notes and documentation accurately and on time. Communicate and collaborate with the hospice care team. Provide grief and bereavement support as needed. Maintain patient confidentiality and follow hospice policies and procedures.
Qualifications:
Bachelor's degree in Theology, Divinity, Religious Studies, Pastoral Care, Counseling, or a related field required. Hospice, healthcare, or spiritual care experience preferred. Compassionate and respectful of different cultures, beliefs, and religions. Reliable, punctual, responsible, and organized. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Valid driver's license and reliable transportation for patient visits.
