Under the general direction and supervision of the Clinical Manager, the Group Facilitator is responsible for providing direct client care. The Group Facilitator also maintains a safe and efficient working and treatment environment per company policies, procedures, and best practices; communicates effectively with the treatment team to ensure safe, quality care is provided to all clients. Group facilitator manages and creates own group topics aligned with the structure of the theme weeks; have independence in choosing their group topics. Group Facilitator can advocate for specialty group populations needed. Group Facilitator is an integral part of the collaborative team, as they provide clinical updates to medical, clinical, and psychiatric providers.
ESSENTIAL JOB FUNCTIONS 1.
Provides direct support services to include engaging clients in and Group Therapy, topics related to various recovery modalities (i.e. 12-Step, SMART, etc), health and wellness promotion, and crisis management. 2. Assist in implementation of treatment plan in collaboration with clinical team. 3. Has responsibility for intake and assessment documentation. Prepares clinical progress notes, event notifications, and other documentation regarding clients in accordance with regulatory requirements and company policies and procedures. 4. Facilitates coordination of care to include case review, therapeutic interventions, and client progress in treatment. 5. Remains flexible to all needs of SRC, including facilitation of group, individual crisis support, and assessments at all SRC facilities. 6. Responsible for ownership of all clinical responsibilities, including collaboration with other agency staff to ensure effective resolution of client, family, insurance, and documentation concerns. 7. Serves as point of contact for coordination of care for all assigned client concerns, crisis intervention, and collaboration with external parties, including client family members, community providers, and other agency members. 8. Maintains availability to facilitate weekend client and/or family group sessions, as well as maintains availability for rotation of weekend assessment coverage. 9. Assigned schedule is dependent on agency needs and may include working at multiple sites and/or schedules to accommodate business need. 10. Minimum of 8 group therapy seesions per week 11. Completes other tasks as assigned and directed. The previous duties are illustrative and not exhaustive. MINIMUM QUALIFICATIONS 1.
High school diploma or equivalent required. 2. A minimum of one year of experience in the substance abuse field; experience and skills in working in substance abuse/behavioral health treatment program a. Or a strong professional reference attesting to a high degree of customer service, punctuality, and professionalism. 3. Current CPR/First Aid Certification. 4. Level 1 Fingerprint Clearance Card in state of
AZ. 5. A
valid driver's license and documentation of appropriate automobile insurance. 6. Peer Support Certified preferred 7. 21 years of age or older.
