Position Summary IMPACT Community Services is seeking a full-time Residential Coordinator (RC). The RC oversees residential services for individuals receiving supports and helps ensure services are safe, person-centered, well documented, and compliant with agency and applicable state requirements. The role coordinates with DSPs, QDDP staff, individuals, families or authorized representatives, and support teams. Essential Duties and Responsibilities Individual Support Conduct placement tours and participate in ISP and individual support-team meetings. Collaborate with QDDP staff on ISP development, reviews, and required documentation. Participate in SIS meetings as required. Maintain current individual records, including face sheets and emergency medical information. Complete required satisfaction surveys and review quarterly documentation for accuracy and timely completion. Residential Compliance Conduct monthly environmental inspections and address identified health, safety, and compliance concerns. Monitor medication administration, health and safety documentation, individual funds, and required residential records. Maintain current Case Books, Provider Books, Individual Books, and other required documentation. Review DSP documentation in Therap and address errors, omissions, and training needs. Monitor billing reports and report identified discrepancies. Ensure incidents are appropriately reported, documented, and followed through. Participate in root-cause-analysis meetings when applicable. Staff Oversight Conduct monthly DSP supervision and maintain required documentation. Provide additional check-ins for DSPs newly assigned to an individual during their first 90 days. Complete annual competency reviews and performance evaluations. Communicate individual needs, documentation expectations, policy updates, and performance concerns to DSPs. Monitor service delivery and address concerns appropriately. Participate in monthly staff meetings and serve as a Human Rights advocate for assigned individuals. On-Call and Emergency Response Participate in the agency's on-call rotation, including occasional weekends. Respond to emergencies, incidents, staffing concerns, and other urgent matters involving assigned individuals or homes. Provide timely direction, communication, and follow-up as needed. Training and Compliance Complete required annual and recurring training, including Human Rights, HCBS Rights, Person-Centered Thinking, Health and Safety, Therapeutic Options, Medication Management, and CPR/First Aid, as applicable. Maintain required training, credentials, and knowledge of agency and DBHDS requirements. Minimum Qualifications High school diploma or GED required; associate or bachelor's degree in human services, psychology, social work, or a related field preferred. At least one year of experience supporting individuals with developmental disabilities or in a related human-services setting preferred. Experience supervising, coaching, or coordinating DSP staff preferred. Knowledge of person-centered practices, ISP requirements, medication management, incident reporting, and residential services preferred. Strong organizational, communication, documentation, and time-management skills. Ability to respond appropriately and professionally to urgent situations. Ability to meet agency requirements for background checks, driving, training, and credentialing. Work Expectations Regular travel between assigned residential homes, attendance at meetings and training, and participation in an on-call rotation are required. The Residential Coordinator is expected to maintain confidentiality, meet established deadlines, communicate effectively, and support the rights, choices, health, and safety of individuals.
