Job Summary Bell Convalescent Hospital is seeking a compassionate, organized, and dependable Social Services Assistant to support our residents, families, and interdisciplinary care team. Under the direction of the Social Services Director, the Social Services Assistant will help coordinate resident services, discharge planning activities, appointments, transportation, durable medical equipment, community resources, and required documentation. This position plays an important role in helping residents and their families navigate the healthcare system while promoting resident dignity, well-being, and a safe transition to the next level of care. The ideal candidate communicates effectively, manages multiple responsibilities, maintains accurate records, and enjoys working with seniors and individuals with medical, cognitive, or physical limitations. Essential Duties and Responsibilities Assist the Social Services Director with resident assessments, care planning, documentation, and follow-up. Communicate with residents and families regarding psychosocial needs, discharge plans, appointments, transportation, and available resources. Help coordinate physician appointments, specialty consultations, transportation arrangements, and related documentation. Assist with obtaining and coordinating durable medical equipment, home health services, medications, and other services needed for discharge. Support discharge planning by communicating with hospitals, health plans, community agencies, assisted living facilities, board-and-care homes, and other healthcare providers. Assist residents and families with applications, referrals, and information related to Medi-Cal, Medicare, Social Security, managed care plans, and community assistance programs. Help identify appropriate placement options and community resources based on each resident's needs. Participate in care plan meetings, discharge planning conferences, and interdisciplinary team meetings as assigned. Document resident and family interactions, referrals, progress, concerns, and follow-up activities accurately and timely. Assist with grievances, resident concerns, behavioral issues, and family communication under the direction of the Social Services Director. Support residents experiencing adjustment difficulties, emotional distress, loss, cognitive impairment, or changes in health status. Maintain resident confidentiality and comply with HIPAA, facility policies, and applicable state and federal regulations. Develop positive working relationships with nursing, rehabilitation, activities, dietary, business office, medical records, and other departments. Perform other social services and administrative duties as assigned. Qualifications High school diploma or equivalent required. College coursework or a degree in social work, psychology, sociology, healthcare administration, or a related field is preferred. Previous experience in a skilled nursing facility, long-term care facility, hospital, assisted living community, or healthcare setting is preferred. Experience working with seniors, individuals with disabilities, or residents with cognitive or behavioral health needs is strongly preferred. Knowledge of Medi-Cal, Medicare, managed care, discharge planning, and community resources is helpful. Strong verbal and written communication skills. Excellent organization, follow-through, and time-management skills. Ability to handle sensitive resident and family concerns with professionalism, patience, and compassion. Ability to work collaboratively as part of an interdisciplinary healthcare team. Basic computer skills and experience with electronic health records are preferred. Bilingual ability is a plus. Physical and Work Requirements Ability to communicate effectively with residents, families, staff, and outside providers. Ability to move throughout the facility and attend resident meetings and care conferences. Ability to work in a fast-paced healthcare environment while maintaining confidentiality and professionalism.
