In person Chaparral House•a non-profit skilled nursing facility Chaparral House is a Bay Area leader in providing person-centered care. We are the only independent, non-sectarian, community based, nonprofit, skilled nursing eldercare community in the Berkeley area. Our compensation is among the highest in the East Bay and we offer one of the best benefit packages in the entire Bay Area. We provide care for up to 49 elders in a dynamic, life-affirming, homelike environment. Activities Director This position establishes a program of activities that is stimulating and that fosters a positive atmosphere through leisure activities that meet the spiritual, cultural, physical, intellectual, and social needs of residents both inside and outside the Community. Essential duties and responsibilities ofActivities Director:
Develop, organize, plan, implement and evaluate a well-rounded and stimulating holistic activity program; which addresses the needs and limitations of residents. Assess the needs of the residents. Observes when a resident's needs have changed and create an activity plan to encompass the changes. Design and post creative monthly bulletin boards and activities calendar. Lead large group activities, e.g. discussion groups, sing-a-longs, and exercise classes. Help manage the facilities volunteer program. Manage staff in the day-to-day performance of their jobs. Adhering to the approved budgets Social Services Assistant The social services assistant main responsibility is to assist in planning, developing, organizing, implementing, evaluating, and directing our Chaparral House's social service department in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis. Essential duties and responsibilities ofSocial Services Assistant:
1. Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility. 2. Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed 3. Participate in discharge planning, development and implementation of social care plans and resident assessments 4. Interview residents/families as necessary and in a private setting. 5. Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident. 6. Call different medical and other service providers to arrange appointments. 7. Arrange transportation for residents. 8. Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care 9. Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.Entry Qualifications:
High school diploma or equivalent. Working knowledge of medical terminology. Proficient in the use of personal computer. Must have 2 to 3 years social services experience. Skilled Nursing Experience required. Some Point Click Care experience preferred.Pay:
