Social Service Assistant Salinas Valley Post Acute Salinas, CA Job Details Full-time $20 - $21 an hour 15 hours ago Benefits Paid holidays Health insurance Dental insurance Paid time off Vision insurance Retirement plan Qualifications High school diploma or GED Full Job Description Social Services Assistant (Full-Time) Job Overview Salinas Valley Post Acute is seeking a compassionate, organized, and dedicated Full-Time Social Services Assistant to join our team. In this role, you will work closely with our Social Services Director to advocate for our residents, support their psychosocial needs, and assist with admission and discharge planning in a skilled nursing environment.
Compensation & Benefits Wage:
$20.00 - $21.00 per hour

Key Responsibilities Assist residents and their families with the facility admission and orientation process. Help coordinate safe, comprehensive discharge planning and community resource referrals. Maintain accurate, up-to-date resident medical records and social service documentation. Conduct regular room visits to assess residents' emotional and psychosocial well-being. Advocate for resident rights and promptly report any grievances to the Director. Assist in organizing resident council meetings and family support initiatives. Qualifications High school diploma required; Associate or Bachelor's degree in Social Work or Psychology preferred. Prior experience in a skilled nursing facility or long-term care setting is highly desirable. Strong written and verbal communication skills in English; bilingual skills are a major plus. High level of empathy, patience, and professionalism when interacting with elderly residents. Proficiency with basic computer programs and electronic health records (EHR) systems.
