Social Services Assistant / Transportation Coordinator Marin Post Acute πŸ“ San Rafael, CA πŸ’΅ $18-$25 per Hour Marin Post Acute is seeking a compassionate, organized, and dependable Social Services Assistant / Transportation Coordinator to join our team. This unique role combines resident advocacy with appointment coordination, helping ensure our residents receive the support and transportation they need to thrive. If you enjoy working with seniors, building relationships, and making a meaningful impact every day, we'd love to meet you! What You'll Do
  • Coordinate and schedule resident transportation for medical appointments, therapies, and outside services
  • Communicate with families, healthcare providers, hospitals, and transportation vendors
  • Assist the Social Services department with resident needs, admissions, discharges, and care planning support
  • Help residents access community resources and services as appropriate
  • Maintain accurate records, schedules, and documentation
  • Provide excellent customer service to residents, families, and visitors
  • Serve as a positive resource and advocate for residents throughout their stay
  • Ensure transportation arrangements are completed efficiently and on time What We're Looking For
  • Previous experience in healthcare, social services, customer service, admissions, or case management preferred
  • Strong organizational and time-management skills
  • Ability to coordinate multiple appointments and schedules simultaneously
  • Excellent communication and interpersonal skills
  • Comfortable working with seniors and their families
  • Computer proficiency and experience with documentation systems preferred
  • Professional, dependable, and compassionate attitude
