Disability Support Coordinator
Job Description
- Help individuals, especially those on the autism spectrum and other developmental disabilities, understand and access essential benefits and resources such as Social Security (SSI/SSDI), Medi-Cal, IHSS, CalFresh, housing programs, transportation services, and other community supports. Advocate
- Work alongside clients and their families to ensure they receive the services, benefits, and resources for which they may qualify. Assist clients in navigating complex systems, overcoming barriers, and communicating effectively with agencies and service providers. Employment & Education Assistance
- Support clients in finding meaningful employment, completing job applications, developing resumes, preparing for interviews, receiving on-site job coaching, communicating with employers, exploring vocational training, and accessing post-secondary education. Help clients develop the skills and confidence necessary for long-term success. In-Home & Daily Living Support
- Assist individuals in developing greater independence through organization, maintaining routines, completing household responsibilities, managing appointments, and building other practical daily living skills while working collaboratively with their families. Monitor Client Goals & Progress
- Regularly track each client's progress toward their individualized goals and objectives. Maintain accurate documentation of services provided, progress made, challenges encountered, and areas requiring continued support. Prepare Progress Reports
- Compile and analyze client progress information to prepare clear, accurate, and professional quarterly, semi-annual, annual, and other required progress reports . Reports should accurately reflect the client's progress toward their established goals, services provided, current needs, and recommendations for continued support. Coordinate With North Bay Regional Center
- Communicate and collaborate with North Bay Regional Center Service Coordinators regarding client progress, services, goals, upcoming meetings, reporting requirements, and changes in client needs. Participate in the coordination of Individual Program Plan (IPP/ISP) meetings and provide requested documentation or progress updates in a timely manner. Maintain Accurate Records & Documentation
- Keep client files, schedules, case documentation, reports, and other required records organized and current. Ensure documentation is completed accurately, professionally, and within established deadlines. Connect Clients to the Right Resources
- Develop and maintain relationships with local agencies, community organizations, employers, schools, healthcare providers, government programs, and other service providers to ensure clients have access to available supports and opportunities. Stay Engaged
- Maintain consistent communication and regular meetings with assigned clients to monitor progress, address challenges, identify changing needs, and adjust support strategies when appropriate. Manage Your Caseload & Schedule
- Organize multiple client schedules, appointments, reporting deadlines, meetings, and follow-up responsibilities while maintaining consistent communication with clients, families, supervisors, and outside agencies.
