You will assist in delivering client-centered care, connecting clients with essential resources, and promoting overall well-being. If you're passionate about making a difference and eager to develop your skills in social work and community outreach, this role is perfect for you! Duties Support in conducting client assessments, case management, and resource identification within social service programs Assist clients by providing information on available services such as Medicaid, Medicare, and workforce development programs Participate in crisis intervention and crisis management efforts to ensure client safety and stability Help coordinate appointments, follow-up care, and documentation to ensure seamless client support Maintain accurate records of client interactions, service plans, and progress notes using professional writing skills Skills Strong problem solving and crisis management strategies Experience working with seniors, individuals with disabilities, or geriatric patients is highly desirable Knowledge of social services programs including Medicaid, Medicare, Section 8 housing, and workforce development initiatives Ability to communicate effectively with clients from diverse backgrounds; bilingual skills are a plus Proficiency in case management practices and resource identification within social service networks Excellent client communication skills combined with motivational interviewing techniques to foster trust and engagement Demonstrated ability to work independently while collaborating effectively within a team environment
Pay:
$20.00 - $25.00 per hour Expected hours: No less than 40.0 per week
