Social Services Assistant Unity Health and Rehab Location:
Miami, FL Weekends $22.00-$25.00/hour Unity Health and Rehab a 250 bed skilled nursing facility located in Miami, FL, is seeking an experienced Social Services Assistant to join our team. As a Social Services Assistant, you will play a vital role in providing exceptional care to our patients and residents. About the
Role:
Assist in coordinating patient care and services, ensuring seamless transitions and communication between healthcare providers, patients, and families. Provide emotional support and empathy to patients, families, and caregivers, helping to alleviate stress and anxiety. Collaborate with the multidisciplinary team to develop and implement individualized care plans, focusing on overall wellness and quality of life. Maintain accurate and up-to-date records, reports, and documentation, adhering to regulatory guidelines and company policies. Participate in quality improvement initiatives, contributing to the continuous enhancement of our services and care delivery.
Responsibilities:
Coordinate patient care and services, ensuring timely and effective delivery of services. Provide emotional support and counseling to patients, families, and caregivers. Participate in care conferences, team meetings, and other collaborative activities. Maintain patient confidentiality and adhere to HIPAA guidelines. Collaborate with other departments, such as Therapy and Rehabilitation, to ensure holistic care. Stay up-to-date with industry developments, best practices, and regulatory requirements.
Requirements:
Minimum of 2 years of experience in a nursing home setting preferred Bilingual English/Spanish preferred Possess strong communication, interpersonal, and organizational skills. Ability to work effectively in a fast-paced environment, managing multiple tasks and priorities. Strong emotional intelligence and empathy, with a passion for supporting patients and families. Ability to maintain confidentiality and adhere to HIPAA guidelines.
