We're looking for a caring and compassionate Social Services Assistant who works in conjunction with the interdisciplinary team to identify and assess our Kupuna's psychosocial, emotional, environmental, spiritual, and support needs to develop a plan of care.Responsibilities include:
Assists with counseling and crisis intervention to facilitate effective coping/problem solving strategies in relation to illness/disability, death, and/or emotional and relationship problems. Makes daily rounds to all units and reviews communication logs, social service logs and loss logs. Completes the process for Social Services to include: initial assessments, state/federal timed assessments, and care plans. Identifies guests who need to Obtain POA, Surrogates or Guardianship and assists in coordinating process to ensure all guests have a representative on admission or within 1 month of admission.Requirements:
Bachelor's degree in Social Work preferred. Membership in National Association of Social Workers and Academy of Certified Social Work preferred. Licensure/ Hawaii State Social Work license or eligible preferred. Minimum of 1 year of experience providing social services or in long term care preferred.The Benefits:
