Job Description

Position Summary:
The Social Services Assistant supports the Social Services Director in planning, organizing, and implementing social service programs to meet the emotional, social, and psychological needs of residents in a 100-bed Skilled Nursing Facility within a CCRC setting. This role ensures compliance with federal, state, and local regulations while promoting resident rights and quality of life.
Key Responsibilities:
Resident Support & Advocacy Assist residents and families with psychosocial needs, care planning, and discharge planning. Provide emotional support and help residents adjust to facility life. Advocate for resident rights and ensure dignity and respect in all interactions. Care Planning & Documentation Participate in interdisciplinary care plan meetings and contribute to assessments. Maintain accurate and timely documentation in compliance with regulatory standards. Assist with MDS (Minimum Data Set) sections related to social services as directed. Family & Community Relations Communicate effectively with families regarding resident care and facility resources. Coordinate family meetings and support groups as needed. Assist in arranging community resources and services for residents. Regulatory Compliance & Quality Assurance Support the Social Services Director in meeting state and federal regulations. Participate in audits and quality improvement initiatives. Maintain confidentiality and adhere to HIPAA guidelines. Team Collaboration Work closely with nursing, therapy, and activities departments to ensure holistic care. Assist in resolving resident concerns and grievances promptly.
