Job Description

We are looking for an experienced Social Services/Admissions Assistant to join our team for our skilled nursing center. The candidate is responsible for assisting the Social Services/Admissions Director in accordance with federal, state and local regulations.
Qualifications:
1-3 years of experience in Rehab or Long Term Care Setting Ability to communicate at all levels Problem solving skills Excellent organizational skills Compassion and desire to work with the elderly Positive attitude Attention to detail Well developed customer service Ability to initiate and foster relationships with medical providers, social workers, discharge planners and individuals working in related field
Responsibilities:
Assist residents and families with social, emotional and family issues Assist care plan meetings with interdisciplinary team, resident and family Assist with discharge planning Assist with facility admissions Provide general information and support to residents, families and general public Make referrals for support services when needed Resident interviews to assist in the completion of required assessments Provide support as needed to
