The Opportunity We are currently seeking a dedicated Social Services Assistant What You Will Do Assist in the planning, organization, implementation, and evaluation of the facility's Social Services Department under the direction of a Licensed Social Worker Ensure social services are delivered in compliance with federal, state, and local guidelines and in alignment with Harborview's values and policies Help develop and monitor individualized care plans for residents, addressing their social and emotional needs Provide ongoing support to residents and families during admissions, transitions, and care conferences Recommend appropriate equipment, services, or referrals for outpatient and home health support Organize and participate in in-service training sessions for the social services team and other staff Advocate for residents and serve as a liaison among staff, families, and external agencies Collaborate with interdisciplinary care teams to ensure holistic, person-centered care What You Must Have Strong interpersonal, communication, and problem-solving skills A resident-first mindset with a deep commitment to dignity, empathy, and respect Ability to manage sensitive situations with professionalism and compassion Benefits - Things we want you to know We offer a comprehensive benefits package, including: 401(k) managed by Merrill Lynch DailyPay Health Insurance with Anthem Blue Cross or Cigna Dental, Life, Vision, and Disability Insurance Paid Time Off Continuing Education Credits Career advancement opportunities!
