Are you bilingual in English and Spanish and looking for meaningful work that allows you to serve families across South Carolina? If so, we'd love to meet you! The South Carolina Primary Health Care Association (SCPHCA) is seeking a compassionate, energetic, and community-focused Outreach Worker to join our South Carolina Agricultural Worker Health Program (AgWHP) . In this rewarding role, you'll help connect agricultural workers and their families with essential healthcare services while making a lasting impact on the health and well-being of one of our state's most vital workforces. Why This Role Matters Every day, South Carolina's agricultural workers provide the food that feeds our communities. Yet many face significant barriers to accessing healthcare. As an Outreach Worker, you will become a trusted advocate by: Connecting farmworkers and their families with healthcare services. Providing health education and resources. Assisting with transportation and care coordination. Building relationships with growers, community organizations, and healthcare providers. Helping improve health outcomes for agricultural worker populations across South Carolina. This isn't just a job—it's an opportunity to change lives. What You'll Do As a valued member of our AgWHP team, you will: Conduct outreach throughout the Midlands, Pee Dee, Upstate, and Lowcountry regions of South Carolina. Educate agricultural workers and their families about available healthcare services. Determine program eligibility and enroll qualified clients. Provide bilingual (English/Spanish) interpretation and translation services. Coordinate transportation to medical appointments. Conduct health education presentations. Perform basic health needs assessments. Connect clients with healthcare providers and follow up to ensure continuity of care. Assist with mobile health clinics and community outreach events. Maintain accurate documentation and enter data into our electronic health system. Support program initiatives and administrative activities that improve access to care.
