Social Services Assistant (Full-Time) Waterview Health & Rehab | Skilled Nursing Facility Let's Be Real… If you've worked in long-term care, you know this role matters. You're the one helping residents adjust, supporting families during tough transitions, and making sure no one falls through the cracks. It's not just paperwork—it's connection, advocacy, and making a difference every day. We're looking for someone who is organized, compassionate, and ready to be part of a team that truly supports both residents and each other. What You'll Do As a Social Services Assistant, you'll work closely with the Social Services Director and interdisciplinary team to support residents throughout their stay. Assist with new admissions and help residents adjust to the facility Support discharge planning and coordinate community resources Document resident interactions, needs, and progress in compliance with regulations Assist with care planning to ensure individualized psychosocial support Advocate for residents' rights and help address concerns Provide support to families during transitions and care planning Assist with daily departmental tasks and projects What We're Looking For Bachelor's degree in Social Work, Human Services, or related field OR One year of supervised social work experience within the last five years Prior experience in long-term care or healthcare preferred Strong communication and organizational skills Ability to work collaboratively with residents, families, and staff Compassionate, patient, and resident-centered approach Why Join Waterview?
