Victorian Post Acute Position Summary Victorian Post Acute is seeking a compassionate, organized, and experienced Social Services Director to lead and oversee the delivery of social services for our residents and their families. The Social Services Director plays a vital role in ensuring residents achieve and maintain the highest practicable level of physical, emotional, and psychosocial well-being while promoting dignity, independence, and quality of life. The ideal candidate must be Social Services (SS) Certified and possess strong leadership, communication, and care coordination skills. This position works closely with residents, families, interdisciplinary teams, and community organizations to ensure comprehensive and resident-centered care. Responsibilities Conduct resident psychosocial assessments and develop care plans. Provide emotional support and counseling to residents and families. Assist residents with healthcare decisions and access to community resources. Coordinate discharge planning and post-discharge services. Maintain accurate and timely social services documentation. Schedule resident appointments and ancillary services. Investigate and resolve resident concerns regarding personal belongings. Collaborate with the interdisciplinary care team to support resident well-being. Build and maintain relationships with community agencies and service providers. Provide staff education and support as needed. Supervise social services staff and ensure regulatory compliance. Qualifications SS Certified (Required) Bachelor's Degree in Social Work, Human Services, or related field. Minimum 2 years of social work experience in a healthcare setting. Long-term care or skilled nursing experience preferred.Benefits Competitive Pay:
