Director of Social Services

Our skilled nursing facility offers residents high-quality care, including long-term care and short-term rehabilitation services. We are currently seeking an experienced and dedicated Director of Social Services to lead our social services team and ensure the well-being of our residents.
Position Summary:
The Director of Social Services will oversee and manage the social services department within our Skilled Nursing Facility. This is a leadership position responsible for developing, implementing, and supervising social service programs for our long-term care and short-term rehabilitation residents.
Responsibilities:
Manage all aspects of the Social Services department, ensuring compliance with federal, state, and local regulations. Supervise, guide, and support Social Services staff to maintain high standards of care. Conduct assessments and develop care plans to address residents' physical, emotional, and social needs. Implement therapeutic programs and interventions for residents. Collaborate with the interdisciplinary care team to provide person-centered care to residents. Advocate for residents' rights and serve as a liaison between residents, families, and staff. Ensure accurate documentation and maintain records according to regulatory standards. Provide crisis intervention, counseling, and support for residents and their families. Participate in care conferences and interdisciplinary meetings related to resident care.
Qualifications:
Bachelor's or master's degree in social work. Current state licensure as a Licensed Social Worker (LSW) or higher. Minimum of 3 years of experience in social services, including at least 2 years in a long-term care (LTC) or short-term rehabilitation (STR) setting. Strong knowledge of regulations governing skilled nursing, LTC, and rehab services. Excellent communication, organizational, and leadership skills. Compassionate, patient-centered approach to care. Ability to work independently and as part of an interdisciplinary team. Proficiency in electronic medical records (EMR) and documentation systems.
