Penta Nascent is hiring a Case Manager (QIDP) to support adults with intellectual and developmental disabilities (IDD) in a residential setting. This role is ideal for someone who wants to build relationships, lead care planning, and see the real-world impact of their work every day. What You'll Do Lead and coordinate care for individuals in our ICF-DD residential program Develop and manage Individual Service Plans (ISPs) Track progress, outcomes, and meaningful life goals Partner with families, guardians, and interdisciplinary teams Ensure compliance with Illinois ICF-DD regulations Prepare documentation for audits and licensing Advocate for residents' independence, dignity, and quality of life What Makes This Role Different Smaller, team-focused environment — not a corporate system Direct impact on individuals' daily lives and long-term success Consistent schedule (no hospital-style burnout) Supportive leadership and paid training Schedule Full-time, 40 hours per week Primarily Monday-Friday daytime hours One Saturday per month required (scheduled in advance) Occasional flexibility for early morning or evening meetings/Care plan meetings, audits, or regulatory needs No overnight shifts Minimum Qualifications (Required to Be Considered) To be eligible for this role, you must meet
