Job Description

A respected post-acute care provider in the southwest suburbs is looking for a Social Services Director to take ownership of resident well-being. This facility places high value on teamwork, integrity, and innovation in resident care.
What You'll Do:
Manage the psychosocial care process for residents from admission to discharge Facilitate care conferences and individualized service plans Guide residents and families through transitions, concerns, and referrals Supervise social services staff and collaborate with interdisciplinary teams Uphold regulatory standards and quality benchmarks
The Ideal Candidate Has:
A Bachelor's degree in Social Work or Human Services field (LSW or LMSW a plus) At least 2 years of experience in long-term care or healthcare social work Deep understanding of discharge planning and case management Empathy, professionalism, and a solutions-focused mindset Prior leadership experience strongly preferred
