Job Summary:

Administrative responsibility for delivery of services through the Social Service Office. To provide social work supervision to staff social workers and to provide necessary back-up when these social workers are unavailable. To participate with Administration and other disciplines in the development of programs, policy, and priority setting regarding Social Services and resident care. To provide direct and indirect service, to identify and meet the medical and non-medical related social and emotional needs of the resident and family at the time of admission, during treatment and care in the facility, and at the time of discharge. Coordinates benefits management, financial and psychosocial planning, discharge planning and external resource referrals.

Duties and Responsibilities:

Responsible for the overall delivery of services through the Social Service Department. Organizes Social Service programs and monitors the delivery, quality and outcome of services. Develops departmental resident care policies. Functions as the Discharge Planning Coordinator for the facility. Remains current with State and Federal Long Term Care regulations. Attends scheduled meetings with the Administrator and weekly Department Head meetings. Assumes administrative role in special projects as requested by the Administrator. Perform other job duties as needed or assigned by the client.

Job Requirements:

Bachelor's or master's degree in social services or related field Two to four years of experience in working with geriatric population, preferably in long term care. Must be Licensed Social Worker (LSW) in the Commonwealth of Massachusetts or Licensed Independent Clinical Social Worker (LICSW) preferred. Fully Vaccinated against Covid-19 Who We Are?
