Skilled Nursing & Long-Term Care Resident Census:

~80 Make a Meaningful Difference — Full-Time, Weekday Schedule St. André Health Care is seeking a full-time Director of Social Services to support the psychosocial well-being of approximately 80 long-term care residents in a collaborative, mission-driven environment. This Monday through Friday role is ideal for a Licensed Social Worker who enjoys leadership, advocacy, interdisciplinary teamwork, and regulatory excellence—without weekend scheduling. What You'll Do Lead Social Services for an 80-resident skilled nursing community Conduct psychosocial assessments and integrate care plans Coordinate discharge planning and community services Advocate for resident rights and family involvement Ensure compliance with Maine and federal regulations Oversee PASRR and nursing level of care requirements Provide staff education and psychosocial guidance Participate in leadership meetings and facility committees Manage departmental operations and budget planning What We're Looking For ✔ Maine Licensed Social Worker (Required) ✔ Bachelor's Degree (Minimum) ✔ 1-3 years of healthcare experience required ✔ Long-term care or skilled nursing experience preferred ✔ Strong understanding of Medicare, MaineCare, and regulatory standards ✔ Experience working with interdisciplinary care teams ✔ Ability to manage services for an 80-resident census Why Join St. André Health Care?
