Human Services Full Job Description Position Summary:
We are seeking a compassionate and organized Social Services Director to join our long-term care team. The Social Services Director is responsible for planning, developing, and directing the overall operation of the Social Services Department to ensure residents receive the necessary emotional support, resources, and services to enhance their quality of life.
Key Responsibilities:
Complete psychosocial assessments for residents upon admission and throughout their stay. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Provide counseling and emotional support to residents and their families. Assist residents and families with discharge planning and transitions of care. Coordinate community resources and referrals as needed. Advocate for resident rights and ensure compliance with state and federal regulations. Participate in care plan meetings and interdisciplinary team meetings. Address resident and family concerns in a timely and professional manner. Maintain accurate documentation in accordance with facility policies.
Qualifications:
Bachelor's degree in Social Work, Human Services, or a related field required (Master's preferred). Previous experience in long-term care or healthcare social services preferred. Knowledge of state and federal long-term care regulations. Strong communication, organizational, and interpersonal skills. Ability to work collaboratively with residents, families, and healthcare staff.
