Duties include but are not limited to:

• Admission and Discharge Planning
• Resident Care Plans and Assessments
• Process Medicaid Application & Follow Up With COMRU
• Ensure that Social Service Componets of MDS are Completed
• Investigate & Review Residents Complaints & Grievances
• Educate & Inform Residents of Their Rights
• Provide Residents & Family Members With Resources In The Community To Meet Their Needs
• Develop & Implement Polices & Procedures For Residents Social & Emotional Needs The idea candidate will have knowledge of rules, regulations and guidelines governing a nursing home.

Must have the ability to work independently when necessary but also a team player when needed.
