GED Full Job Description Director of Social Services Position Summary:

The Director of Social Services is responsible for supporting residents and families throughout their stay by assessing psychosocial needs, coordinating care planning, facilitating discharge planning, and connecting residents with appropriate community resources and services.

Key Responsibilities:

Complete social service assessments for residents upon admission and throughout their stay. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Lead and participate in resident care plan meetings. Communicate regularly with residents and family members regarding needs, concerns, goals, and discharge plans. Coordinate discharge planning for residents returning home or transitioning to another healthcare setting. Arrange and coordinate home health services, durable medical equipment, and other post-discharge resources. Assist residents and families with adjustment, coping, and access to community support services. Advocate for resident rights and promote resident well-being and quality of life. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Collaborate with nursing, therapy, admissions, and other departments to ensure seamless transitions of care.

Qualifications:

Previous experience in healthcare, long-term care, social services, case management, or a related field preferred. Discharge planning experience strongly preferred. Strong communication, organizational, and interpersonal skills. Ability to work effectively with residents, families, staff, and outside agencies. Knowledge of community resources and post-acute care services preferred. High school diploma or equivalent required; no degree required.
