Social Services Director Responsibilities:

Oversee all aspects of social services in the facility, ensuring residents' psychosocial needs are met. Assist residents and their families with admissions, discharge planning, and transitions. Provide emotional support and counseling to residents and families as needed. Advocate for residents' rights and help resolve concerns or conflicts. Develop and implement individualized care plans in coordination with the interdisciplinary team. Maintain compliance with federal, state, and local regulations. Coordinate social and community resources to enhance resident care.

Social Services Director Qualifications:

LSW in New Jersey Experience in healthcare or long term care setting. Strong understanding of federal and state regulations, including Medicare and Medicaid requirements.
