A skilled nursing facility in the Kingston NY, area is seeking an experienced Director of Social Work (DSW) to lead our Social Services department and support resident well-being in a collaborative, supportive environment.

Responsibilities:

Oversee all Social Services operations Lead discharge planning and care coordination Support residents, families, and interdisciplinary teams Ensure compliance with state and federal regulations Mentor and develop Social Services staff

Qualifications:

Bachelor's Degree in Social Work or related field (MSW preferred) Leadership experience in a skilled nursing or healthcare setting (required) Knowledge of Medicare, Medicaid, and discharge planning Strong communication and organizational skills
