If you are passionate about advocating for residents, supporting families, and helping create a positive quality of life for those we serve, we would love to hear from you. ? What You'll Do Serve as an advocate for residents and their rights, needs, and preferences Develop meaningful relationships with residents and their families Assist residents with psychosocial needs, adjustment to the facility, and transition planning Coordinate and participate in care plan meetings and family conferences Maintain accurate and timely social services documentation Assist with admissions, discharge planning, and transitions of care Address resident and family concerns and help facilitate appropriate resolutions Connect residents and families with community resources and services Collaborate with nursing, therapy, activities, dietary, and other departments to provide person-centered care Promote a welcoming, supportive, and respectful environment for residents What We're Looking For Experience in social services, preferably in a skilled nursing or long-term care setting Bachelor's degree in Social Work, Psychology, Sociology, or a related field preferred Strong communication and interpersonal skills Compassionate and resident-focused approach Excellent organization and follow-through Ability to work effectively with residents, families, and an interdisciplinary team Knowledge of long-term care regulations and resident rights is a plus ?
