What You'll Do:

Support residents and families during transitions and throughout care Create social histories, assessments, and care plans Drive admissions, tours, and discharge planning Collaborate with the care team and external agencies Keep documentation timely and meaningful Be the go-to for social needs, referrals, and resources

What You Bring:
