Summary We are seeking a compassionate and skilled Social Worker to join our team. In this role, you will play a vital part in enhancing the quality of life for our residents by providing essential support and resources. This position is crucial in ensuring that our residents receive the best possible care tailored to their individual needs. Responsibilities Conduct assessments and develop individualized care plans for residents. Provide counseling and support to residents and their families during difficult times. Facilitate group therapy sessions to promote emotional well-being. Collaborate with healthcare teams to ensure comprehensive care delivery. Manage case files and maintain accurate medical records as required. Assist residents in accessing community resources and services. Implement crisis management strategies when necessary. Requirements Valid state licensure as a Social Worker is preferred but not required. Experience in clinical counseling or case management preferred. Strong communication and interpersonal skills.
