You plan for the resident's discharge ensuring they reach their full potential.
You Will:
Support individual needs, preferences, routines, concerns, and choices Arrange needed adaptive equipment, clothing, personal items, and referrals to outside agencies Coordinate social services with the management team Document in the patient clinical record Communicate with and encourage the family to participate in care planning
You Currently:
Undergraduate degree in Social Work required. Promote resident rights, dignity, and confidentiality standards Problem-solve creatively, are organized, and flexible. Prefer an associate or bachelor's degree and meet state certification requirements. Have one to two years working in geriatrics Display excellent written and oral communication skills
