Job Description:
Develop and maintain business relationships with Case Managers, Nursing Homes, Hospitals, Physicians, and other referral sources. Facilitate referral appointments for patients being admitted to hospice Willingness to conduct in-services in community facilities Willingness to attend networking events
Additional Requirements:
Excellent organizational and time management skills. Professional appearance and attire. Willingness to travel, with a valid CA driver license and good driving record. Ability to work with a variety of healthcare professionals across a wide variety of settings
