Adheres to all required documentation requirements. This is a Temporary Full-Time position.
Requirements:
Criminal Background Investigation:
Must be able to pass the California State Department of Justice criminal background investigation, Live Scan, and/or fingerprinting.
Education:
High School Diploma required. Bachelor's Degree in Social Work, Psychology, or a related field preferred.
Experience:
With High School Diploma:
At least six years of experience in the field of mental health/social services, with a minimum of three years working with homeless mentally ill adults who are dually diagnosed with mental illness and substance abuse.
With Bachelor's Degree:
At least four years of experience in the field of mental health/social services, with a minimum of three years working with homeless mentally ill adults who are dually diagnosed with mental illness and substance abuse.
Special Skills & Knowledge:
Ability to relate to individuals with severe impairments in community functioning. Ability to respond quickly, effectively, and calmly to member crises and life situations. Knowledge of Fair Housing and Employment Laws to advocate for clients to sustain housing. Ability to identify, collaborate, and network with property owners and management companies. Knowledge of HACLA, HACOLA, and DMH's Housing Division. Knowledge of the Los Angeles County Department of Mental Health's Rehabilitation Option model of service provision and documentation requirements. Basic computer skills and literacy required to use the agency's electronic health record system. Must be able to log on, type, use a mouse, and navigate various screens or windows to complete daily chart documentation. Knowledge of the South Central Los Angeles Community or experience with inner-city environments. Ability to work as a team member and collaborate with other agencies, community resources, and members' families. Valid California Driver's License and availability of a car with adequate insurance. Knowledge of social rehabilitation philosophy.
Duties:
Develop and locate housing resources appropriate for formerly homeless mentally ill TAY, Adults, and Older Adults. Housing options should include Section 8 and low-income housing. Assist clients in completing rental agreements, Housing Authority forms, and any other required paperwork. Ensure clients attend housing appointments and Housing Authority meetings as needed. Develop relationships with landlords and property owners to identify new housing resources for clients. Explain how BAFMA services will help clients become stable tenants and collaborate with landlords to secure housing for clients, including negotiating sublease agreements and client service participation addendums to lease agreements. Work with the Director of
MHSA-FSP
Program to identify properties for purchase or lease to provide affordable independent community housing for clients. Coordinate all placements with the Personal Services Coordinator, ensuring a good match between the client and the housing. Secure agreement regarding move-in arrangements in consultation with the client and the Personal Services Coordinator. Develop resources for temporary housing while homeless clients prepare for permanent housing, such as shelters, transitional housing, sober living, residential rehabilitation programs, and drug treatment programs. Monitor clients placed in permanent independent housing situations to identify potential problems and develop intervention strategies with the Case Manager, key staff in the
MHSA-FSP
program, and the landlord/property owner. Structure the financial aspects of clients' move-ins to subleased properties. Develop money management plans with clients to ensure rents are paid, and move-in and security deposit costs are addressed. Screen and prepare pre-authorization requests for Shelter Plus Care (S+C) certificates for families and individuals. Educate case management staff to assist in the completion of HACLA and
HACOLA S+C
Quarterly Client Progress Surveys. Complete 6-month Housing Assessments for DMH. Involve members in all aspects of the housing services they receive as partners, providing them with choices and opportunities for empowerment whenever possible. Prepare for weekly clinical staff meetings with current information on the status of clients and housing resources. Attend all inter/intra-agency meetings as scheduled or assigned. Provide mental health rehabilitation services such as counseling, money management, social skills training, and other independent living skills as needed by assigned members, adjunctive to the Personal Services Coordinator to ensure successful placement. Complete all Rehab Option, Outcomes Study, and other paperwork within prescribed timeframes. Maintain a UOS log and/or COS log daily, documenting all services to be billed and attaching the progress notes for these services. Service codes and time billed must be consistent on the UOS log and progress note. Submit documentation to the supervisor in a timely manner (weekly). Bill at least five hours of services daily. Work as a team with FSP 2 and other BAFMA staff to provide consistent services. Enter client information into HMIS (Homeless Management Information System) for LAHSA-HUD, documenting clients' stability in housing and milestones in recovery. Complete Annual Progress Reports (APR) on clients at Hoover Street and Figueroa Properties. Work with property management companies to understand the dynamics of our FSP Adult clients. Keep separate Housing Charts/Records and participate in Housing Audits. Utilize DMH Medical Records in support of housing as needed. Perform other duties as assigned.

$31.15 - $32.31 per hour
