DESCRIPTION POSITION SUMMARY
The Hospital/Community Liaison (HCL) is responsible for building and maintaining relationships with hospitals, nursing homes, rehabilitation facilities and the greater community to market the services of The Overlook Health Center to generate referrals and increase admissions. The HCL is responsible for meeting and or exceeding referral and census goals. The HCL must be highly organized, strategy driven, flexible and possess the ability to understand and act accordingly to market variables. The HCL must have the ability to interact with all customers, both internal and external with the professionalism and diplomacy necessary to portray The Overlook Health Center in a positive manner. Must possess superlative communication and interpersonal skills across all levels while maintaining sensitivity to patient/provider needs. The HCL must present in a professional and positive manner in all venues, events and functions.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Relationship Management and Lead Development Maintain existing accounts with site visits and develop new relationships with case managers, physicians, nurses, senior centers, elder law attorneys and other professional and community referral sources. Identify and overcome barriers to referrals and address any obstacles to achieving results. Expand customer base by constantly identifying potential new accounts. Gather and use market data to maximize potential referral sources and increase sales Conduct extensive research on assigned territory and develop and implement plans and goals to cultivate new business with key stakeholder groups and nurture existing accounts, in pursuit of meeting weekly, monthly and annual referral goals. Follow future trends in territory and respond preemptively to market changes. Develop and implement a marketing plan, in conjunction with management. Coordinate and attend related trade shows, conferences, and community events for marketing purposes Ensure The Overlook Health and Rehabilitation Center is represented at appropriate events and functions. Attend sales and marketing meetings as directed. Liaison Activities Monitor and respond to referral alerts from multiple referral management systems in a timely manner that facilitates the acceptance of a bed offer to result in an admission. Provide daily updates to hospitals and other referral sources regarding bed availability. Review screens to determine if the referral meets SNF requirements. Ensure units have all necessary equipment and required authorizations prior to admission Coordinate with Case Managers and Social Workers on transitioning referral to accepting facility Meet with referral patients to answer any questions regarding admission process Provide clinical evaluations and placement navigation for The Overlook and its affiliates (including skilled nursing, post-acute, home health care, enhanced living, and assisted living) Per the direction of the administrator or her/his designee, perform job duties not specified herein.
QUALIFICATIONS
Bachelor's Degree (preferably in marketing or health care related field) Current license in Massachusetts as LCSW, LICSW, LPN, or RN desirable Experience in long term care screening, home care, hospice and/or sales experience a plus Minimum of five years of Sales and Marketing experience working in a health care field Valid MA driver's license and reliable transportation
