Job Description
The Youth Support Specialist provides youth-guided, strengths-based, and recovery-oriented support to children, youth, transition-age youth, and families receiving Behavioral Health Home Services. The position promotes youth voice, self-advocacy, wellness, natural supports, engagement in services, and connection to community-based resources. The Youth Support Specialist works as part of a multidisciplinary Behavioral Health Home team and collaborates with care coordinators, clinical staff, nurse care managers, psychiatric consultants, family members, caregivers, schools, medical providers, community partners, and other natural supports as appropriate and authorized.
