Job Description
AsterCare Home Health LLC is seeking a motivated, professional, and relationship-driven Community Liaison to help grow our presence and build strong referral relationships throughout the communities we serve. This is primarily a field-based community outreach and relationship-development position . The Community Liaison will establish and maintain professional relationships with physicians and physician offices, hospitals, skilled nursing facilities, assisted living communities, rehabilitation facilities, senior communities, and other healthcare and community organizations. The primary goal of this position is to increase awareness of AsterCare's services, educate healthcare professionals about appropriate home health referrals, develop long-term referral relationships, and help connect eligible patients with the home health services they need. What We're Looking For We are looking for someone who is self-motivated, outgoing, organized, and comfortable developing new professional relationships face-to-face . The successful candidate should enjoy being in the community, visiting physician offices and healthcare facilities, introducing our agency, following up consistently, and building relationships over time. This is not primarily a desk or office-based position . The ideal candidate will be comfortable independently planning daily outreach activities, identifying new opportunities, maintaining existing relationships, and representing AsterCare professionally throughout the community. Existing healthcare relationships are highly valued, but we are also willing to consider a motivated candidate who demonstrates strong communication, networking, and relationship-building abilities. Key Responsibilities Develop and maintain professional relationships with physicians, physician offices, hospitals, skilled nursing facilities, assisted living facilities, rehabilitation centers, senior communities, and other potential referral sources. Conduct regular in-person visits with existing and prospective referral sources. Educate physicians, office staff, discharge planners, case managers, social workers, and other healthcare professionals about AsterCare's home health services and referral process. Identify opportunities for appropriate home health referrals and new community partnerships. Develop and carry out outreach strategies to increase awareness of AsterCare Home Health. Maintain consistent communication and professional follow-up with referral sources. Build long-term relationships rather than focusing solely on individual referrals. Represent AsterCare at healthcare meetings, community events, networking opportunities, and other appropriate outreach activities. Monitor referral activity and provide regular updates regarding outreach efforts, opportunities, and market feedback. Work closely with clinical and administrative staff to help facilitate a smooth transition from referral to admission. Maintain ethical, professional, and compliant relationships with referral sources in accordance with applicable Medicare and healthcare regulations. The Community Liaison will spend a significant portion of the workday in the community developing and maintaining referral relationships.
