Do you enjoy working in clinical sales and business development? If so, Bridges Health is seeking a dedicated Post-Acute / Community Liaison to join our team and support post-acute communities and healthcare providers in Oklahoma. We're looking for a confident sales person who values service, collaboration, and clinical excellence. In this high-impact role, you will: Assessed patient care needs and facilitated smooth transitions across the continuum of care, partnering with hospitals, physicians, home health, and hospice providers to secure appropriate placement in skilled nursing, long-term care, assisted living, and independent living settings Serve as a clinical expert and trusted resource to patients, families, and hospital / healthcare based referral partners Build and maintain strong relationships with hospitals, LTACs, healthcare providers and community based referral partners Support our mission of delivering compassionate, patient-centered care Key Responsibilities Build and maintain strong relationships with hospital referral sources, including case managers and physicians. Facilitate business development with hospitals, LTACs, and physician groups to ensure a consistent pipeline of patient referrals. Conduct pre-admission screenings and assessments to determine clinical, equipment, and transportation needs. Collaborate with facility Admissions Coordinators to ensure a smooth, patient-centered admission experience. Serve as an educational resource for patients, families, and healthcare professionals throughout the referral and admission process. Maintain accurate records of daily activity and referral source interactions within the referral tracking system. Required Experience & Qualifications Current and unencumbered Oklahoma
LPN / RN / RRT
license preferred Minimum of 1 year of experience as a hospital and / or community based sales person Experience in a skilled nursing or skilled rehabilitation setting preferred Background in clinical sales or business development Reliable transportation for daily travel Benefits We offer competitive pay relative to experience.
