Our mission is to enhance the quality of life for individuals by delivering exceptional healthcare services and emotional support tailored to meet each patient's unique needs. Role Description This is a full-time hybrid role for a Hospice Community Liaison based in Topeka, KS allowing for both in-office and work-from-home flexibility. The Hospice Community Liaison will be responsible for building and maintaining strong relationships with healthcare providers, community organizations, and referral sources. This role covers the greater Topeka, Kansas Metro,requiring regular engagement with healthcare providers, facilities, and referral sources throughout the territory. Daily tasks include engaging in community outreach, educating individuals on hospice and home care services, and supporting families and patients in accessing care. The role also involves identifying opportunities for partnership and fostering a positive image for the organization within the community. Qualifications Skills in Community Engagement and Community Outreach to foster relationships with stakeholders and establish organizational presence. Strong abilities in Communication and Customer Service to effectively interact with patients, families, and professional partners. Understanding of Home Care and hospice services , including the ability to educate community members about their benefits. Proven ability to build and maintain professional relationships with referral sources. Established book of business and/or established professional relationships within the healthcare community, including physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral sources, required. Self-motivation and capacity to work in a hybrid environment, balancing in-person and remote responsibilities.- Professional experience in healthcare, marketing, or related fields preferred but not required. Access to reliable transportation and a valid driver's license for local travel within the Topeka Metropolitan Area.
