Job Description
Performs minimally complex social service work. Responsible for authorizing services, over site of services and evaluating the effectiveness of authorized services. Works under moderate supervision with latitude for the use of initiative and independent judgment.
Assesses client's needs and authorizes services to meet the identified needs.
Coordinates service delivery.
Develops Person Directed Plan.
Provides Crisis Prevention and Management services when needed.
