Full Time Shift:
Mon-Fri, Daytime Wages:
$20/hr
DOE Summary:
This position is responsible for planning, implementing and collaborating housing services for individuals and families who are homeless or are experiencing homelessness. Responsibilities Carry out intake interview and evaluation of need with households Screen households' circumstances for monetary assistance and/or program eligibility and provide qualifying households with appropriate resources or referrals. In conjunction with clients, develop and monitor individualized housing stability plans which include goals, action steps, and timelines Coordinate linkages with other agencies on behalf of clients to facilitate fluid service delivery, maintain collaborative program planning, service coordination and shared resources Establish and maintain working relationships with rental agencies and landlords Participate in outreach activities to landlords, service providers, and community groups Maintain case files, document services provided, and progress accomplished per grant requirements Maintain reporting learn and maintain the State reporting system HMIS (Homeless Management Information System) file for each participant. Maintain current and confidential files Attend weekly staff meetings Perform other duties as assigned Requirements 2 Year experience with and commitment to working with homeless and/or low-income households Highschool Graduate or general education diploma associate's degree preferred Ability to plan and organize for self and others, be a guide, teacher, and role model Strong organizational and record keeping skills Knowledge and experience related to several of the following: crisis intervention, domestic violence, homelessness, child development, mental health, case management, substance abuse Ability to network with other providers of similar services Willing to travel as needed Driver's license and insurance, ability to pass a criminal background check Any combination of experience and training which provides the applicant with the knowledge, skills, and ability required to perform the work
