Job Summary The Coordinated Services Team (CST) Care Coordinator will have a comprehensive knowledge of the human service system; have knowledge of how to access area resources; be skilled in written and oral communication; and be able to successfully facilitate groups and work well with a divergent group of people. The CST Coordinator will provide case management and crisis services to youth and their families experiencing mental health services. Duties may include non-standard work weeks. As an employee of a local government agency, qualified employees may be eligible for the federal Public Service Loan Forgiveness (PSLF) program, which may provide student loan forgiveness for borrowers who meet program requirements. Key Responsibilities Identify and bring together a multi-disciplinary team of people that will collaboratively work with the youth and family. Together with team partners, conduct a comprehensive and multi-dimensional summary of strengths and needs of the youth and family. Schedule and facilitate team meetings to complete the summary of strengths and needs and review the results. Ensure completion of corresponding paperwork. Ensure the development of a safety plan for each youth to address potential crisis situations at home, in the community, and/or at school Develop crisis plans utilizing the strength of the youth/family and available community resources to resolve the immediate crisis. Facilitate follow-up interventions with youth/family. Together with team partners, develop the Plan of Care, specifically outlining each team member's responsibility, timeline for accomplishment, and outcome expectations. Ensure the Plan of Care is amended as necessary to meet the changing needs of the youth, family, service providers, and community. Coordinate implementation of the Plan of Care and monitor ongoing delivery of services. This responsibility includes regular contact with the youth, family, and service providers. Schedule and facilitate regularly scheduled team meetings to monitor the plan as a team. Make intervention and assessment decisions using the "Best Practice" philosophy. Work collaboratively with law enforcement, families, other county departments, schools, acute care services providers, and other community resources to coordinate appropriate crisis resolution and referral.? Communicate pertinent information on a timely basis and participate in clinical supervision case reviews, staffing's, and/or case planning meetings. Complete all necessary health care/medical and financial record documentation required to maintain complete medical and financial billing records. Complete and coordinate appropriate and timely medical record documentation, which may include assessments, follow-up contacts, crisis plans, administrative, demographic, insurance, and financial information. All other duties as assigned, including responding to an emergency event.

Supervision Received:

Receives supervision from the Crisis Supervisor assigned to Ongoing Crisis. Qualifications Bachelor's degree from an accredited college or university in social work, human services, psychology, or related degree is required. A minimum of one (1) year of experience providing mental health or crisis services is preferred. Experience working with youth with mental health issues is preferred. Prompt and regular attendance. Successful completion (within three months after beginning position) of the Crisis Program training which is provided as part of on-the-job training. Valid Wisconsin driver's license and automobile insurance are required. Salary is based on experience and education.
