HolistiCare 5014 W Northern Ave Glendale, AZ 85301 HolistiCare is seeking a compassionate and dependable Arizona Licensed Professional Counselor to join our outpatient behavioral health team two days per week . Responsibilities Provide individual and group counseling Complete assessments and individualized treatment plans Maintain timely and accurate clinical documentation Provide trauma-informed and culturally responsive care Coordinate services with the clinical team Follow AHCCCS, ADHS, HIPAA, and agency requirements Qualifications Active Arizona LPC license in good standing Master's degree in counseling or a related field Current Arizona Fingerprint Clearance Card Professional liability insurance Experience with substance-use and co-occurring disorders preferred Knowledge of AHCCCS documentation preferred
