Job Description

WHO WE ARE
Warriors Heart is a Joint Commission-accredited, DBHDS-licensed residential treatment center dedicated exclusively to veterans, active-duty military, and first responders. We exist because those who serve others often struggle to ask for help themselves. Our multidisciplinary team delivers evidence-informed, trauma-sensitive addiction and co-occurring disorder treatment in a community built on warrior culture, brotherhood and sisterhood, and a shared commitment to recovery.
THE ROLE
We are seeking a skilled, mission-driven SUD Counselor to join our clinical team. In this role, you will carry an active caseload of warriors in residential treatment, providing individual counseling, co-facilitating group programming, collaborating with a tight-knit interdisciplinary team, and guiding clients through treatment from intake to discharge. You will work in a structured residential setting where no two days are the same and your clinical skills genuinely matter. This position requires scheduling flexibility; to include evening, and one weekend day availability consistent with our program schedule and client census. Duties to Include but not limited to: Carry an individual caseload and provide regular one-on-one counseling sessions focused on substance use, recovery, and related challenges. Complete Biopsychosocial Assessments within 24 hours of client intake. Develop and maintain individualized Treatment Plans and Discharge Plans within 5 days of assignment; update plans as the client progresses through treatment. Facilitate group sessions including 12 step, relapse prevention, psychoeducation, life skills, and process groups. Document all clinical interactions, notes, assessments, and plans in our electronic health record within established timeframes. Attend and actively contribute to weekly multidisciplinary Treatment Team meetings. Coordinate care with the client's Dual Diagnosis Therapist, Nurse Practitioner, and Psychiatrist. Communicate with family members and loved ones (with appropriate releases) and track all family contact. Build and finalize an individualized Aftercare Plan no later than five days prior to discharge. Complete Discharge Summaries within 48 hours of client departure. Uphold Warriors Heart's documentation compliance standards, confidentiality policies, and Joint Commission accreditation requirements.
